 I have over 23 years of Total contact compression care with over a few  hundred custom made devices for deep third degree Burns. I specialize in  severe face and neck third degree burns.  I also design total contact  splinting for third degree burns of the extremities (knee, elbow,  shoulder, hand, joints).
A Third degree burn is not equal in density  to a second degree burn. A third degree burn is deeper and can reach the  muscle.  The graph skin that is transplanted over the burn area will  not have the same elasticity as normal skin. Once it heals, the scar  process will start to take place within the first week and progress to  layer itself over and over as it does not know when to stop  (Epitheliaztion). The deeper the burn the more it will layer. Areas of  the face like the mouth and chin\jaw area will tend to get thicker and  be layered more than areas like the forehead. This will cause thick  hypertrophic scars, scar bands on the neck. The contraction will start  to pull the surrounding areas causing severe deformity. The hypertrophic  scar will start to take away range of motion of the chin, neck leaving a  still freeze position of the head and neck.  On the face it will cause  functions like the inability to open the mouth sufficiently to eat a  normal meal. I also make a dynamic intra-oral and extra-oral microstomia  orthosis to control the shrinking of the mouth.  The scar tissue can  also pull the skin surrounding areas near the eyes which will contribute  to the inability to close the eye lids. Burn nasal channel can also  constrict the opening of the nose making patient developed nasal  obstruction which will make it hard for patient to breath. I control  this condition with a nasal stent device. The graph skin over a deep  third degree burn on the facial/neck has different variables that  contribute to how the scar is going to behave. 

The compression treatment for third degreee  burn's resulting in hypertropic scars with a  Total Contact device has shown great results in my experience for the past 25 years. (Facemask for Burns)
